Infertility problem occurs due to hormonal imbalance and can be fixed with empiric therapy which can balance the hormone levels. This therapy differs from person to person based on their problem. Andrologist can help people suffering from genetic disorders.
Assisted Reproductive techniques
ARTs are used when infertility treatment fails or is not available. In this, you can get pregnant without having sex. Based on the cause and specific type of infertility, the urologist may suggest the following treatments.
Intrauterine Insemination (IUI)
IUI is good for low sperm count, retrograde ejaculation, movement problems and other causes of infertility. In this technique, the doctor places the sperm into the female partner’s uterus through a tube.
Intracytoplasmic Sperm Injection (ICSI)
This treatment is used to treat severe male infertility. This technique is applied when you have very poor semen quality. It is also used when there is no sperm in the semen. Sperm is taken from the testicles or epididymis by surgery for this treatment. The treatment takes place by injecting a single sperm into the egg with tiny needles. The egg is moved to the uterus once it is fertilized.
In Vitro Fertilization (IVF)
It is a treatment where the egg of a female partner or donor is joined with the sperm in the lab. In IVF the ovaries are made overly stimulated by drugs which help to retrieve the matured eggs. The fertilized egg is put back into the uterus after three to five days of growth. It is mostly used in women whose fallopian tubes are blocked. But is commonly used in cases where the man has a low sperm count.
Sperm Retrieval for ART
Many microsurgical methods can remove the blockage of sperm. The goal here is to get large no. of cells which are of the best quality. This is done by not harming the reproductive tract. This method can improve male fertility to a large extent. The methods are as follows:
Testicular Sperm Extraction (TESE)
It is used to diagnose the cause of azoospermia by getting enough tissue for sperm extraction. The sperm extracted from the testicle can be used frozen or fresh. In this technique, small biopsies are often done.
Percutaneous Epididymal Sperm Aspiration (PESA)
This is a cost-effective technique where you get no surgical cuts. It is done under general or local anesthesia. In this method, the urologist sticks a needle attached to a syringe into the epididymis and gently withdraws the fluid.
Testicular Fine Needle Aspiration (TFNA)
It is used to collect sperm from the testicles. A syringe and needle penetrate the scrotal skin to pull sperm from the testicles.
Microsurgical Epididymal Sperm Aspiration (MESA)
MESA uses a surgical microscope and yields high amounts of motile sperm. The sperm can be kept in a frozen state to use it later for IVF treatment.
